  • First Name, Middle Initial (optional), Last Name - Please list name you preferred to be called.
  • Include title and its associated department.
  • Briefly list your primary role and responsibilities at the College of Public Health.
  • This section will help others know what questions you can answer or support you can provide on specific topics, such as travel authorities, information about study abroad, information about health benefits, how to purchase something, etc. -- Please briefly share a shortlist of things you can help a member of the CPH community (faculty, staff, student, alumni) with. -- Example: "When to contact Lauren Baggett: Promoting published research, promoting a new program or event, using the CPH brand, with questions about CPH communications and marketing strategy."
  • Please select your primary office location.
  • Min. size requirements: 500x500 pixels; 72 dpi. This can also be emailed to cphnews@uga.edu.

  • List your visiting hours for students; secondary office location; and/or how to book an appointment. (eg - email or youcanbook.me).
